"What you need to do" for 7th grade and Department of Health requirements BELOW.
Summary:
Hawai'i law requires all students to complete a physical examination, age-appropriate immunizations, and tuberculosis (TB) clearance before they can attend childcare, preschool, or K-12 schools.
Parents must submit a completed health record or an official appointment notice by the first day of school, otherwise children will be prohibited from attending until these mandates are met.
These requirements apply to all students entering school for the first time, those moving into the seventh grade, and those transferring from out-of-state.
While medical or religious exemptions are available with the correct documentation from a healthcare provider or the school, no other exceptions are permitted by the state.
